"The lead CIA operative during the 1961 Bay of Pigs invasion accidentally fired on disguised B-26 bombers flown by U.S.-backed Cuban exiles, according to declassified documents of the failed bid to topple Fidel Castro. Some of the CIA-provided bombers, which were made to look like Cuban military planes, were hit by 'friendly fire' from 75mm recoilless rifles and 55mm machine guns on a transport boat."
